Community Memorial Healthcare participates in the Providence Health Information Exchange. This
partnership gives your healthcare providers access to the latest, most complete details about your health.

We Protect Your Information - Here's How

We follow all State and Federal privacy laws, including the Health Insurance Portability and
Accountability Act (HIPAA), which protects the confidentiality and security of health related information. All healthcare providers who participate in the Providence Health Information Exchange agree to comply with written policies and procedures controlling access to your health information. We have security measures in place to prevent any unauthorized access. These measures include, but are not limited to, encryption, firewalls, password protection, and the ability to track and audit all user activity within your records.

We Respect Your Choice

Your health information will automatically be included in the Providence Health Information Exchange. If you do not wish to share your information, you must opt-out by completing the Opt-out Form. If you choose to opt-out, you may opt back in at any time. Opt-out Forms are available at www.ProvShare.org in the “For Patients” section.

Community Memorial Healthcare Clinical Data Exchange

Prior to May 3, 2025, Community Memorial Healthcare's Clinical Data Exchange allowed you to permit your health information to be shared by participating medical groups, hospitals, labs, radiology centers, and other health care providers through secure, electronic means. The Community Memorial Healthcare Clinical Data Exchange includes Meditech (hospital services) and AllScripts (outpatient services).

Your participation in Community Memorial Healthcare Clinical Data Exchange is voluntary and subject to your right to opt-out. Your receipt of treatment or health plan coverage for treatment will not be conditioned on whether or not you choose to exercise this right.

Unless you opt-out, any authorized healthcare provider who participates in Community Memorial Healthcare Clinical Data Exchange, or is a member of a health information exchange that is connected to a Community Memorial Healthcare Clinical Data Exchange service, can electronically access and share your health information through Community Memorial Clinical Data Exchange.
